It's difficult to measure how x-ray imaging has transformed dentistry since its use became prominent a half century ago. As equipment and methods standardized, the technology revolutionized the way we diagnose tooth decay and other mouth-related issues.
One of the more useful of these methods is called the bitewing x-ray. The term comes from the shape of the device a patient holds between their teeth with the film attached on the side toward their tongue. We direct the x-ray beam to the outside of the patient's cheek, where it passes through the teeth to expose on the film. Its particular design provides clearer images since the patient's bite helps keep the film still and distortion-free, making it easier to view signs of early tooth decay.
Bitewing x-rays usually consist of four films, two on either side of the mouth, necessary to capture all of the teeth (children with smaller jaws, however, often only require one film per side). How frequently they're conducted depends on a number of factors, including the patient's age: children or young adolescents are usually filmed more frequently than adults, usually every six to twelve months. Frequency also depends on a patient's particular decay risk — the higher the risk the more frequent the x-ray.
Regardless of how often they're performed, a similar application principle applies with bitewing x-rays as with any other radiological method: As Low As Reasonably Achievable (ALARA). With the ALARA principle in other words, we're looking for that sweet spot where we're able to detect the earliest stages of dental disease with the least amount of radiation exposure.
Bitewings fit this principle well: a patient receives only a fraction of the radiation exposure from a four-film bitewing as they do from a daily dose of environmental radiation. Factor in new digital technology that reduces exposure rates and bitewings pose virtually no health risk to patients, especially if conducted in a prudent manner.
The benefits are well worth it. Thanks to bitewing x-rays we may be able to diagnose decay early and stop it before it causes you or your family member extensive tooth damage.
Do-It-Yourself (DIY) is a deeply held American trait for building, renovating or repairing things without the services of a professional. The Internet has only made this tradition easier: There are scores of videos showing people how to do things on their own like build a deck, fix a dryer or bake an award-winning soufflé.
But some things are best left to the experts, which if you tried to do using too little knowledge or a lot less training could turn out disastrous. A prime example is becoming your own orthodontist and using dubious home methods to straighten your teeth. If that sounds preposterous, the American Association of Orthodontists recently reported it does happen, with one in ten of their members saying they have treated patients who attempted their own smile-straightening projects.
Often found on social media, these methods usually involve household items like rubber bands or dental floss to straighten teeth. Like other forms of DIY, the object is to save money. In the end, though, these self-orthodontic methods could result in dental damage that could cost much more to repair (if indeed it's repairable) than what might have been spent with professional orthodontics in the first place.
Utilizing extensive training, experience and artistry, orthodontists work with the mouth's natural ability to move teeth in a precise manner for a planned outcome. They carefully consider each individual patient's jaw and facial structures, along with the severity and complexity of their bite problem, as they design and implement a treatment plan involving braces, clear aligners or other orthodontic appliances.
A rigged homemade device to move teeth can't adequately take these factors into account. As a result, you may be risking permanent gum and bone damage—and you may even lose teeth in the process. Even if repairable, such damage could require oral surgery, cosmetic dentistry or more extensive orthodontic procedures.
In the end, you're highly unlikely to be successful at DIY orthodontics—and you won't save any money. A healthy and beautiful smile is well worth the cost of professional, high-quality orthodontics.
Think you’re too old to have your teeth straightened? In reality, healthy teeth can be moved at any age to better positions. For the many adults who have some form of malocclusion (bad bite), orthodontics is still a viable option even in later years.
As important as it is to self-image and confidence, treating misaligned teeth can benefit you more than just improving your smile. Misaligned teeth may be harder to keep clean, setting up a mouth environment advantageous to the development of tooth decay or periodontal (gum) disease, the risks of both rising with age. And normally aligned teeth are easier to chew with than those misaligned.
Age isn’t the determining factor for whether you’re a good candidate for orthodontics — but your dental health is. Gum disease in particular can cause supporting bone loss, which can complicate orthodontic treatment. It’s important then that we first perform a complete oral examination and attempt to treat problems such as decay or gum disease first before attempting tooth movement.
What type of orthodontic treatment you’ll need will depend on the type of malocclusion you have and its relationship to the way your jaws fit together. Because your adult jaws have fully developed you may need orthognathic (“jaw straightening”) surgery to address certain advanced forms of malocclusion. If your bite problems aren’t that severe (the majority of situations) they can be treated with braces or, an increasingly popular alternative, clear aligners. These customized clear plastic trays are nearly invisible compared to metal braces and are effective for most patients.
Following the completion of tooth movement and other bite procedures, you will most likely need to wear a retainer to help prevent the teeth from reverting to their older positions. You may need to wear the retainer for a longer period than a younger patient, or perhaps indefinitely. Even with this mild inconvenience, though, you’ll still experience the positive effects of healthier and better functioning teeth and a great new smile.
